To submit a successful claim to your insurance provider or Medicaid network, you must gather the following three pieces of documentation:

Request a physical prescription from your treating oncologist, dermatologist, or primary physician. The prescription must specifically read: "Cranial Prosthesis for medical hair loss" (do not let them write "wig"). -
An Approved ICD-10 Diagnosis Code
Ensure your doctor includes a formal medical diagnosis code directly on your prescription paperwork. Common eligible codes include:- L63.9 – Alopecia Areata, unspecified
- Z51.11 – Encounter for antineoplastic chemotherapy
- L65.9 – Noninflammatory dermatological hair loss
